This is a Service Improvement Project to improve the already existing CPA document using by MDT (multidisciplinary team – including Community nurse, care managers, family of individual, carers, individual to discuss their care. Finding out from them their needs and what they wish to develop. I's about person centredness approach, inclusiveness. 

So my task is to come up with an improved easy read CPA suitable for individuals with learning disabilities not mental health. 

3. Meaning the document will be formatted in a way that's easy to read with pictures to aid understanding. 
Remember this is based on the NHS Change Model as a framework for service improvement (NHS Institute for Innovation and Improvement 2008).
This project won't be implemented.
To use Process Mapping and you are expected to use the PDSA (Plan, Do, Study, Act) cycle in the production of your project plan.

Highlight the challenges one faces implementing this project , was everyone on board with the idea.
