  There are many forms of treatment for Post-Traumatic Stress Disorder (PTSD).  The treatment option I would deploy in a disaster situation is therapy.  According to our text there a two types of therapy options cognitive behavior and mindfulness.  Cognitive behavior is used to treat cases of self-blame and PTSD. Mindfulness is a subset of cognitive therapy.  These practices and identification of self-blame help the patients manage though processes leading to increased amounts of self-blame and shame (King, et al., 2013). Cognitive behavioral therapy is focused on helping individuals manage their own thought process, recalling negative experiences as a means of understanding the impacts those recollections have on the ways in which blame is assigned (Matthews, 2008).  Mindfulness helps the patient by coming to understand ones own thought process, patients suffering from acute anxiety are able to arrive at a place of acceptance of their thoughts leading to the ability to function in the presence of negative thoughts and tremendous amounts of self-blame.  These techniques help does affected by PTSD to heal from within without the use of outside sources like drugs.  I never felt as if drugs should be the answer but I understand the need for them.  These drugs can also become a negative aspect of a persons life.  We can develop long-term tolerance (needing more of the drugs to achieve the same effect) and withdrawal symptoms (physical effects and discomfort when substance use stops).In children they can negatively impact a childs social and emotional growth as well as their ability to learn.

There are several treatment options for individuals who have suffered from a disaster situation whether man-made, natural, or a hybrid of each. Different treatment options could include therapy-based options, such as, cognitive behavior therapy and mindfulness therapy. There is also a pharmacological intervention and antidepressant therapy. There are several factors that need to be taken into consideration after a disaster event with individuals who are trying to cope with the situation. Some of these factors are the nature of the disaster, the severity or frequency of the disaster, the loss or trauma manifested from the disaster, the social and environmental issues, support systems, stress reaction, coping with dramatic change, the physiological issues, along with memory impairment, and different emotional issues such as depression and increased anxiety. With each factor being identified, coping can be difficult and should have a set plan of action due to each individuals coping behaviors being different.

The most popular and or appropriate choice for individuals struggling from PTSD, post-traumatic stress disorder, are the different treatment options noted above, cognitive behavior therapy, mindfulness therapy, pharmacological intervention, and antidepressant therapy. Cognitive behavior therapy is the most commonly used out of each of the treatment options for PTSD. Cognitive behavior therapy is used to help individuals stop identifying negative thinking and or self-blame. Cognitive therapys main goal is to redirect and or manage negative thoughts by recalling the experience and understanding where the associated blame is going. This type of therapy allows patients to feel self-aware and allows them to feel more in control of their thoughts. Mindfulness therapy is similar to cognitive behavioral therapy. Relaxation techniques are used in order to slow down or calm an individuals mind. When a patient relaxes and reduces their anxiety, they are able to dig deeper into their mind in order to draw out different thought patterns. By doing this, the patient begins to diminish their anxiety and push out negative thoughts or self-doubt or self-blame like cognitive therapy. Pharmacological intervention is not only beneficial for those suffering with PTSD, but it can help in the early onset of PTSD after a disaster event. Different drugs are used with pharmacological intervention that block the glutamatergic activity and the glucocorticoid receptor prior to trauma. Most therapies that involve drugs are dependent on the individual being treated and their emotional response. Antidepressant therapy was most widely used to treat patients with PSTD, however, treatment throughout the years with this therapy has been inconsistent. Although antidepressant therapy gives an immediate relief to its patients, there are long term effects such as sleep issues, issues with cognitive behavior, balance issues, and reduced awareness to include own mortality, (American Public University). With these issues and the unpredictability of this type of therapy, PTSD should be treated with cognitive behavioral therapy first in order to start treatment in a more beneficial manner.

In psychiatry and mental health, nurses and prescribers work tirelessly with patients and present them with the best alternatives to manage their conditions. These plans of care are designed with the patient’s best interest in mind; according to Melnyk and Fineout-Overholt (2018), patient-centered care is an approach that increases patient engagement in their healthcare and leads to a better perception of healthcare outcomes. Since the facility I work for treats patients with acute psychiatric disorders that require emergency stabilization, the practitioner and the nurses could sometimes take the initial treatment measures without the patient’s consent.

For this week’s discussion, the issue that comes to my mind is the involuntary treatment of patients with episodes of psychosis in the psychiatric ward. Sometimes a patient refuses acute psychiatric treatment, but the multidisciplinary team determines that the patient will benefit from being medicated and has obtained a legal proxy to accept treatment for the patient. We can still honor the patient’s values by listening to the patient express their feelings and concerns about the medication therapy; sometimes, the patient may have had a bad experience with side effects. They can inform you what has worked for them and what has not. Preserving the patient’s preference plays a significant role in future compliance and overall outcomes; according to Hoffman et al. (2014), including the patient’s values and preferences in implementing EBP in their treatment plan is crucial for its success.

Patients can sometimes be unable to make informed choices about their care and will need a healthcare surrogate (Sisti, 2017). Determining a patient’s ability to consent to treatment is a skill only psychiatrists are legally entitled to in my facility. Still, nurses can advocate for the patients by communicating and identifying those alternatives that were effective for them in the past.

Patient Preferences and Decision Making

It is the obligation to offer quality medical services to patients. One way to ensure quality health service is practicing patient-centered care (Russo et al., 2019). Traditionally, physicians and other healthcare providers entirely made decisions regarding patients’ health. Currently, the implementation of evidence-based practice has encouraged healthcare providers to involve patients in the decision-making process (Melnyk & Fineout-Overholt, 2018). If at all, the patient should make an informed choice, and healthcare workers are obliged to respect patients’ preferences.

 During my rotations, I came across a 23-year-old female patient struggling with weight control. On physical examination, the patient had a calculated body mass index of 27. She had elevated cholesterol on the lipid profile test. According to the Ottawa Hospital Research Institute’s Decision aids, the patient was informed of three available treatment options, including weight loss and observing diet control, pharmacological drugs for obesity, and surgery. The patient was advised that the best option was weight loss and diet control while doing a serial lipid test to assess her lipid levels. The patient chose to use a diet plan and do regular exercises. She added that she would prefer bicycle riding and swimming routinely. Fortunately, when the patient returned for the next visit, she had lost about 3kg. Dealing with obesity is a patient’s responsibility, and they should be involved in the decision-making process on how to control their weight (Jackson et al., 2020). Involving the patient in the decision-making was essential because she took it as her responsibility to reduce her weight.

I chose obesity decision aid, which offers three options for treating obesity, including weight loss and diet control, use of medications, and bariatric surgery. The decision aid provides options from which the patient can choose their preference, as illustrated in my experience during my rotations. As a future registered nurse, I would use the decision aid while giving health education to obese patients or individuals struggling with weight control. Additionally, I would use the decision aid to help me maintain a healthy weight by exercising regularly and eating a healthy diet to prevent being overweight.

More than half of the worlds population lives in Southeast Asia. Also, the region is one of the largest producers of goods and services. A good amount of those products are transported through the Strait of Malacca. Cargo flow through the Straight accounts for twenty five per cent of all cargo transported on global waters. The city state of Singapore is lucky enough to be located on the pathways of this busy trade route. According to Sinay (2021) Port of Singapore is known as a top maritime capital and amongst the Top 10 Smart Ports Around the World. The port is connected to more than 600 ports in 123 countries and employs about 200,000 workers. It also transships half of the worlds supply of crude oil. Finally, it is known as the busiest transshipment port in the world (Sinay 2021). I dont think we can find any port in the world busier than the port of Singapore. The government of the city state, investors, and the port management are doing great job managing the flow of cargo through their terminals. 835th Transportation Battalion of Surface Deployment and Distribution Command (SDDC) has a detachment in Singapore. As an SDDC Battalion Mobility officer, I witnessed the amount of work being done at the Singapore ports first hand. It is simply mind-blowing. 

Singapore is not only doing great with the cargo management but also making it much bigger by building the Tuas mega-port. Ship Technology (2020) states that Tuas mega-port expansion is projected to be completed by 2040 Consolidating container handling at the site, it will improve efficiency and vessel turnaround times, enabling the port to handle 65m TEU annually, compared with 45m at its four existing box terminals. Is attracts 130,000 vessel calls on average a year. (Ship Technology 2020)

To accommodate an increase in cargo volume Tuas terminal will include the next-generation vessel traffic management system able to predict possible congestion and assist vessel route planning. Remote controlled vessel pilotage, sense-making system will be employed to avoid congestion and improve the efficiency of port operations with multi-million TEU flow annually.

Good day all,

Ports have a variety of ways they can accommodate for the increased cargo, but they do have limitations. One limitation that comes to mind is space, a port can only go so far before they have to start stacking cargo on top of one another, like most do already. Another issue that some ports are facing is they they are not within the easiest route to get to. For instance, the Savannah Port (in the state of Georgia) is one of the ports on the East Coast that can take cargo ships from the West Coast, but because it would mean the ships moving and utilizing more fuel than they anticipated, and they also are not used to the routes to get there, it would be easier for the cargo ships to wait their turn to be unloaded. Some ways for the ports to fix these problems is to purchase more land, purchase higher cranes, and start having incentives that are worth the extra fuel prices to move from one side of the country to the other. What these incentives could be, I am not sure as I have never worked in a port, but can only assume something like fifty percent off the unloading fee would be beneficial, but I am not sur if it would be beneficial enough to move a boat from one side of the United States to the other. We could also look at opening up the smaller ports to help with the large cargo ships, but we would be limited on space and only certain vessels would be able to cross there. Maybe we could even start opening up passenger ports to product ports.
