Medical Office Management

Foundational information on the profession of medical assisting. You learned about the required characteristics of a medical assistant, the history of the profession, ethic and legal obligations of the medical assistant, patient communication, and telephone techniques. Part A: Kayla Christianson, CMA, has been employed six years by the cardiology practice of three physicians. She is a graduate of a CAAHEP-accredited school. Furthermore, Kayla received extensive hands-on training performing ECGs while doing her required externship.

 

Kayla has completed an ECG ordered by Dr. Hsu for Mrs. Warner, a 76-year-old patient. Dr. Hsu, Kayla’s boss, has telephoned her explaining that he was behind schedule doing rounds at the hospital. He asked her to do him a favor and interpret Mrs. Warner’s ECG, sign his name, and fax the report to Mrs. Warner’s referring internist who is expecting the results.

 

Provide detailed answers for each of the following questions. Your response should be two (2) pages in length.

 

• Given the scope of Kayla’s education, training, and years of experience as a CMA, would this favor fall within the AAMA guidelines of her responsibilities? 

 

• Would any portion of Dr. Hsu’s request fall within the guidelines? If so, which portion(s)? Is an exception to these guidelines ever allowed? 

 

• How should Kayla respond to Dr. Hsu?

 

Part B: For more than two years, medical assistant Linda Lewis had been employed by Drs. Norek and Klein, who are gerontologists. Also on staff are two registered nurses, a medical laboratory technician, and a medical social worker. The daughter of one of the doctor’s patients has just called the office. She is very distraught at the seemingly diminished capacity of her mother and insists on speaking to the doctor.

 

Linda explains that both physicians take only emergency calls during patient appointment hours, but that she will take a detailed message. This caller, however, suggests that not only should her call be considered an emergency, but that she will sue the doctor if the call is not handled accordingly.

 

Provide detailed answers for each of the following questions. Your response should be two (2) pages in length.

 

• What should Linda do immediately to diffuse the situation?

 

• Is this clearly a case when the call should be passed on to one of the registered nurses or the medical social worker?

 

• Is this a case when the physician should be called to the telephone because of the threat of an impending suit?

 

• How could Linda ascertain whether or not this is indeed an emergency? Is it up to her, as a medical assistant, to make such a determination?

 

• Because this is the patient’s daughter rather than the patient herself, does Linda have any reason to enter into a conversation with the caller? Could Linda be ethically bound by confidentiality not to admit the woman’s mother is a patient?

GOAL:  Construct an application or presentation that demonstrates synthesis of the key concepts in population based nursing, leadership, and management.

 

ASSIGNMENT ACTIVITIES AND SUGGESTED TIME FRAMES AND GUIDELINES:

 

1. The capstone project allows the student to apply knowledge learned throughout the BSN program with a focus on Leadership and Population Health. The student should choose a topic in nursing with a focus on leadership, community health nursing, or a combination of both.  

 

2. Note:  This project is tied to the practicum and therefore is time-limited, as is the clinical practicum, to a time frame of 9-10 weeks.

 

3. To successfully complete your project, the faculty strongly suggests that you follow the protocol suggested below.

a. BY THE END OF LESSON 1: Meet with your Preceptor to discuss his or her views on a proposed project for you to work on for his or her site.  Ask the Preceptor, “What would you like to see me do for you?” Meet with your Course Faculty (CF) to discuss what you would like to do and reach an agreement on the project to be completed. Remember the preference is for one identified within the needs for the site and your RN-BSN preparation skill set.

b. BY THE END OF LESSON 1-2: Learn about your practicum site. Synthesize and set priorities focusing on procedures, the population, the staff, and collecting information related to your project. Begin collection of information for your literature review.

c. BY THE END OF LESSON 3: Write your introduction with questions. Send this to your CF for critique. If you desire, schedule a meeting with the CF to discuss the review of your draft (in fact, the CF would prefer to meet with you).

d. BY THE END OF LESSON 4: Begin to develop your class, PowerPoint, or other materials you need to carry out your project.

e. BY THE END OF LESSON 5: Plan your activity, presentation, or the appropriate method to share your project with the practicum site. The CF should review and approve your materials prior to their use.

f. BY THE END OF LESSON 6: Present to your Preceptor or to the team or to whomever has been identified as the recipient of your work. Don’t forget to have an evaluation by the participants if you do a presentation.

Project Overview

In this project you will essentially go through the stages of project management (i.e. Defining the project scope, Project time, project cost, human resources, project communications, project risk, and project procurement) for a case study given to you. You will be using the MS project management system for the project. Your project grade will be based on your analysis and quality of final report.

 

Project Description

Assume you are the director of external affairs for a national not-for-profit medical research center that does research on diseases related to aging. The center’s work depends on funding from multiple sources, including the general public, individual estates, and grants from corporations, foundations, and federal government.

 

Your department prepares an annual report of center’s accomplishments and financial status for the board of directors. It is mostly text with a few charts and tables, all black and white, with a simple cover. It is voluminous and pretty dry reading. It is inexpensive to produce other than the effort to put together the content, which requires time to request and expedite information from the center’s other departments.

 

At the last board meeting, the board members suggested the annual report be “upscaled” into a document that could be used for marketing and promotional purposes. They want you to mail the next annual report to the center’s stakeholders, past donors, and targeted high-potential future donors. The board feels that such a document is needed to get the center “in the same league” with other large not-for-profit organizations with which it feels it competes for donations and funds. The board feels that the annual report could be used to inform these stakeholders about the advances the center is making in its research efforts and its strong fiscal management for effectively using the funding and donations it receives.

 

You will need to produce a shorter, simpler, easy-to-read annual report that shows the benefits of the center’s research and the impact on people’s lives. You will include pictures from various hospitals, clinics and long-term care facilities that are using the results of the center’s research. You also will include testimonials from patients and families who have benefited from the center’s research. The report must be “eye-catching.” It needs to be multi color, contain a lot of pictures and easy-to-understand graphics, and be written in a style that can be understood by the average adult potential donor.

 

This is significant undertaking for your department, which includes three other staff members. You will have to contract out some of the activities and may have to travel to several medical facilities around the country to take photos and get testimonials. You will also need to put the design, printing, and distribution out to bid to various contractors to submit proposals and prices to you. You estimate that approximately 50,000 copies need to be printed and mailed.

 

It is now April 1. The board asks you to come to its next meeting on May 15 to present a detailed plan, schedule, and budget for how you will complete the project. The board wants the annual report “in the mail” by November 15, so potential donors will receive it around the holiday season when they may be in a “giving mood.” The center’s fiscal year ends September 30 and its financial statements should be available by October 15. However, the nonfinancial information for the report can start to be pulled together right after the May 15 board meeting.

 

Fortunately, you are taking a project management course, at Empire State College, and see this as an opportunity to apply what you have been learning. You know that this is a big project and that the board has high expectations. You want to be sure you meet their expectations, and get them to approve the budget that you need for this project. However, they will only do that if they are confident that you have a detailed plan for how you will get it all done and a well prepared estimated budget. You and your staff have six weeks to prepare a plan that you will present to the board on May 15.
