Deliverable 4 – Code Of Ethics Intake Packet

Competency

Assess current legal and ethical principles and the application of such principles in healthcare practice.

 

Instructions

You have recently been promoted to Health Services Manager at Three Mountains Regional Hospital, a small hospital located in a mid-size city in the Midwest. Three Mountains is a general medical and surgical facility with 400 beds. Last year there were approximately 62,000 emergency visits and 15,000 admissions. More than 6,000 outpatient and 10,000 inpatient surgeries were performed.

 

 

Three Mountains was recently in the news after employee emails were published in which patients were discussed in very negative terms, although patient names were not used. The employees that sent these emails were fired. The CEO and the Board of Directors has tasked you with developing an intake packet for new patients that will reestablish patient trust in the facility, employees, and staff. The patient packet will address new patient concerns by including information about HIPAA, informed consent, a confidential health history report, and a living will. The new packet will also include the values of the organization and a code of ethics.

 

 

Your intake packet will start with the Hospital's Code of Ethics that includes at least 10 ethical points. The code of ethics should include (at a minimum):

  • 1)Patient relations
  •  2)Physician activities
  •  3) Billing for services
  •  4) Political activities
  •  5) Conflict of interest
  •  6) Communication, including social media)
  •   7) Privacy

 

The Code of Ethics should be submitted in bullet format in a Word document with an introduction. APA formatting for the ethical points and proper grammar is required.

Upon completion of your code of ethics, please prepare a PowerPoint presentation (or other shareable Webware/software you prefer) with narration in which you explain each ethical point and its relevance in the healthcare industry with examples to the CEO, so that she can answer questions from the Board.

  1. The PowerPoint should have a minimum of 5 slides and a maximum of 12.
  2. Describe each ethical point in the Code of Ethics.
  3. Use the notes area on each slide as needed to expand on the key points.
  4. You may use a free screen capture site such as Screencast-O-Matic to record a video of your presentation. Screencast-O-Matic is a site and program that can perform screen desk and audio capture up to 15 minutes for free, and can be utilized on a Windows or Mac computer. (Note: You can use another, similar program if you prefer. Screencast-O-Matic is only a suggestion). Make sure that both your voice and the PowerPoint slides are captured on the video.

Your audio presentation should include an introduction, a concise discussion of each slide, and a conclusion. The presentation should demonstrate your overall knowledge of the content, pronunciation of words, organization, proper recording, professionalism, and clarity.

Your presentation should be 10 minutes or less. Be sure to include the following:

  • Include a link to the location of your live video on the last slide of your PowerPoint presentation.
  • Make sure to use audience specific language and tone in your PowerPoint. Remember, you would be presenting this to the CEO of your facility.

EDiscovery Guidelines

This  week we are going even further into the legalities of health  information and  the focus is eDiscovery. eDiscovery refers to  electronic data that is sought  for the use of evidence in a legal  proceeding. As you all know, technology is  heavily used in the  provision and documentation of healthcare. This means that  electronic  data is being utilized as evidence more often now then it was when   healthcare facilities were keeping mostly paper health records. An HIM   professional works with these related challenges:

 

  • How  do we ensure that our organization is adequately preserving data?
  • How  do we properly identify data sources that may contain information that is  pertinent to eDiscovery?
  • How  do we ensure that there is appropriate access and security of data that is  being sought as evidence in eDiscovery? 

In this  assignment, please create Guidelines for eDiscovery for a  current day  healthcare facility. Use your text, supplemental resources  available on this  topic at AHIMA.org and resources available in the  Rasmussen College Library to  guide your research.

Your  submission must contain a list of numbered guidelines that are  supported by  professional resources evidenced in citations. Do not copy  from your resources  but rather create original guidelines and support  them with resource material  via citations. The average submission will  be no more than 2 pages plus a  resource page.

Addressing A Decrease In Mammography Volume

SCENE ONE: The Vice President of Clinical Services reviewed a  report from radiology and noticed a decrease in the volume of  mammography studies performed over the past 9 months. She wants to learn  what may be contributing to the decrease, so she arranges to meet with  the Radiology Performance Improvement Team (R-PIT) which monitors the  performance of the radiology department and works on improving services.

 

  1. As the lead on R-PIT, you begin by completing the R-PIT Plan of  Action which will be presented to the VP of Clinical Services for  approval. Copy the R-PIT table below into a Word Document and complete  it for submission.

SCENE TWO: Six months later (after the R-PIT was approved,  quality measure researched and radiology quality improvement action has  been completed) you request a new data report to validate mammography  improvements. The new data report demonstrates a new problem,  unfortunately, and this problem is outside of the radiology department.  You find that of the 1800 mammograms completed in radiology over the  last 3 months, only 27% of them have been coded and therefore have not  been billed. This leaves a little over 1300 mammograms unbilled which is  likely why multiple patients have called recently asking why they have  not been billed. 
 

You went to high school with the coding manager, Charmaine, and head  over to her office to share this information. Charmaine asks you about  the Process Improvement Team and you offer to assist with the newly  discovered Coding Delay Issue. Charmaine is committed to HIM best  practices for her entire coding team, including the outpatient clinic  coding group.

  3.    Consider Charmaine's intention of functioning with Best  Practices. Create a Best Practices Coding Statement for Charmaine to use  for her own Performance Improvement Team in HIM. Best Practice  considerations should include at a minimum – patient satisfaction,  financial well-being of the facility and chart completion requirements  by regulating agencies such as CMS (Centers for Medicaid and Medicare  Services).

Defining The Project

The key project objectives are outlined and you've defined your role  as a project manager. Next, you'll need to further the project  definition in preparation for the project charter. Consider the initial  scenario for Sidney Park Health System, in addition to the following  information:  Sidney  Park Health System is experiencing a shortage of coders. Over  the past five years, there has been a 30% vacancy rate for coders  throughout the main hospital and satellite clinics within the healthcare  system. The backlog and  vacant positions has created an issue related  to unbilled revenue, overworked coders and extended work hours and  schedules. It is estimated that there is an  annual loss of $12 million.  It has also been communicated that many of the qualified coders that  were offered a position declined because they are interested in working  from home. To address the problem, the health information management  department is proposing that there be an implementation of a home-based  coding system. This concept is feasible, given that Sidney Park Health  System utilizes an electronic health record and all patient information  is available electronically. You have been assigned as the project  manager to  oversee the feasibility and implementation of this project.
