This assignment has two deliverables.  The first deliverable is a powerpoint presentation where you create and present a portfolio for your investor.  Please submit both your powerpoint slide deck and a video of your presentation (line will be needed) . The second deliverable is a written summary where you first describe your investor and then describe which theory you applied for the portfolio.

IRB process

The Institutional Review Board (IRB) determination at the University, has the goal to assist students in the preparation for transition into the NR 705 course, Project and Practicum II. The IRB is an administrative body established to protect the rights and welfare of the subjects participating in research or project activities conducted under the auspices of the institution with which it is affiliated (Harris, Roussel, Dearman, & Thomas, 2020). For the majority of students in the DNP program, a full IRB review will not be required.

Depiction of gender stereotypes in Arthurian Lore

I had no idea that the tales of Arthur and his knights of the round table were so extensive, having only been exposed to Arthurian lore through modern film and television. I initially found the Old English vocabulary quite difficult to follow, but eventually was able to read between the lines so to speak.

Once again, the depiction of men and different is like the mythological tales of different eras we have already covered. The events leading up to Arthurs conception and birth begin with a powerful man, Uther Pendragon lusting after another mans wife. Rightfully so, the Dukes wife Ingraine refuses to lay with Uther and counsels her husband that they should flee to protect her honor (Malory, 33). This invokes the wrath of Uther, and he sets out to make war with the Duke. Ultimately, through some clever magic, Ingraine is tricked into sleeping with Uther, believing him to be her husband the Duke. This shows woman being portrayed as mere sexual objects for the desires of powerful men who abuse their power.

The conception of Galahad plays out in a similar fashion, but I noticed some differences regarding the portrayal of men and woman. King Pelles desired that Lancelot should have sex with his daughter Elaine, that he may begat a child upon her. And fain would King Pelles have found the mean to have had Sir Launcelot to have lain by his daughter, fair Elaine. (Malory, 501). The subtle difference I noticed here was King Pelles was counselled by another woman in how to achieve his aims of Lancelot impregnating his daughter Elaine. The Dame Brisen came forward to explain that Lancelot only loved Guenever and for him to sleep with Elaine he would have to believe it was in fact Guenever (Malory, 502). And so it came to pass that Lancelot was fooled into sleeping with Elaine, believing it was Guenever. Upon waking and discovering the ruse, he was furious and shameful, and desired to murder Elaine for her deception.

Much of the Arthurian tales depict men as having to overcome great battles and earn honor and praise through their heroic deeds, waited upon hand and foot by their many maidens, while the woman are portrayed as mere pawns. Of course, many of these battles are fought over woman. One could argue that this portrays woman as the root cause of all war and conflict.

I have not yet been involved in the implementation of a new evidence-based practice intervention however, there is an intervention that would be useful in my workplace in distinguishing between viral and bacterial infectious etiologies while aiding in antibiotic stewardship, which is the use of procalcitonin testing. It has been estimated that 30%-85% of antibiotic prescriptions are unnecessary or inappropriate and even when indicated, often exceed recommended durations, leading to ever increasing antibiotic resistance. 

Procalcitonin testing has been proven to be approximately 70% accurate in distinguishing bacterial from viral pathogen and when used in adjunct to algorithms and clinical judgement, has been shown to reduce unnecessary antibiotic use by up to 50% without increasing mortality or morbidity (Rhee and Mansour, 2022). A scenario where this may be useful is adding procalcitonin testing to ED order set that may be applied to presentations where there is high variability between viral vs bacterial etiologies, such as respiratory illnesses. 

The implementation of evidence-based practice (EBP) follows a process that includes cultivating a spirit of inquiry, asking a clinical question, searching for evidence, appraising, and applying the evidence, and evaluating its application in clinical practice (Abu-Baker et al, 2021). The initiation of the use of procalcitonin would follow a process that includes modifying the implementation for the available resources such as initially using the testing in the ED to establish type of infection but not in the guidance of antibiotic use as part of a multiphase rollout process, establishing EHR order sets, and keeping patient centered goals in the forefront, which may include anticipating shorter hospital stays with reduced risk of hospital acquired infections. The most difficult stage is the implementation stage, which may be accomplished by time-intensive strategies, various models, staff education, computerized decision support, performance feedback, multiprofessional collaboration, media campaigns, and resources, which are often minimal (Tucker & Gallagher-Ford, 2019). Education and proving cost-effectiveness would be key in the implementation of procalcitonin testing in small hospitals such as the one at which I am employed, and thus, I would add a thorough cost analysis with projected savings both immediately and in the future.
